Below are links to download the worksheet you'll need to fill out and return to us at least 24 hours in advance of your consultation. Please download the one that matches the type of situation you're in, and email it to welcome@dubois.law. There is a lot of detail, but we only ask that you make a solid effort to put most of it in--we certainly don't expect perfection. Names, addresses and telephone numbers of relevant family members, and a list of the assets we'll be helping you protect, with ballpark values for each. The more info we have the better we can plan!
